Request for Device Analysis
Shipping Instructions

International Rectifier and FedEx have teamed up to provide you with an easy, time definite way to return your components for Device Analysis, utilizing the RDA Form. The following instructions are the preferred method for returning a small number of devices (40 maximum) and are not to be used for the return of entire shipments or batches of devices. These should be handled through the normal Return Material Authorization (RMA) process.

Selecting the Packaging If your samples weigh 0.5 pounds (0.227 Kg) or less, use the FedEx Letter envelope. If it weighs more than 0.5 pounds, please use the FedEx Pak.

Package Preparation Be sure that the package has enough cushioning material to provide for safe transport. If the devices are ESD sensitive, please ensure they are properly protected by using normal ESD shipping and handling precautions.

NOTE: Before sealing the envelope, place the completed Request for Device Analysis (RDA) form in the FedEx Letter or Pak.

Completing the Air Way Bill (AWB) Please complete the sections that are listed below.

IMPORTANT: Be sure to check the page titled "Where to Send the Devices". Different items have different destinations.

Section 1 Please fill in today's date, your name, your phone number (very important!), your company name and complete address.

Section 2 To process your request as quickly as possible please fill in the RDA number found in the upper right corner of the Request for Device Analysis Form.

For devices sent to Italy, fill in the following: Commodity/Description: "Failed component for Failure Analysis only", and Country of Manufacture: Italy

A customs value must be declared for all shipments. Please multiply the number of items being shipped by $0.25 USD each and write that amount in the space provided for Customs Value. Example: 17 items multiplied by $0.25 each = $4.25.

NOTE: The maximum value per shipment is $10.00 USD, or a maximum of 40 rejected parts per package. You must also enter a weight. If you are using a FedEx Letter, please write in 0.5 pounds or 0.227 Kg. If you are using a Pak, write in the actual weight, starting at 1 pound going up in 1 pound increments.
