Completing the Commercial Invoice for International Shipments
Each international shipment must be accompanied by a Commercial Invoice.
Please complete the following:
1. Fill in the AWB# corresponding with this shipment. For FedEx customers, fill in the FedEx Tracking Number (AWB#) printed on the FedEx label corresponding with this shipment.
2. Under "Date of Exportation" Fill in today's date.
3. In section marked "Export References" fill in the RDA number. The RDA number can be found in the upper right corner of the Request for Device Analysis form.
4. Under Shipper/Exporter, write in your name, your company name, and your complete address and phone number.
4.1 Under Consignee, write in address of the appropriate Device Analysis Center where the devices are being sent.
5. Fill in the country of Export.
6. Write in the type of packaging used with your carrier. FedEx customers indicate either FedEx Letter or FedEx Pak.
6.1 For devices sent to Temecula or El Segundo, in the country of manufacture write USA, For devices sent to Italy write Italy
6.2 Write in country of destination.
7. Under "Qty" fill in the number of items being returned.
7.1 Fill in full description of goods as follows:
US shipment: Transistor, American Goods Returned, HTS# 9801.00.1098
Italian shipment: Failed component, returned for Analysis only
For Europe:
US shipment: Failed component returned for analysis only
7.2 Unit of measure is (ea)
8. Fill in the total value of the shipment. (Number of pcs. being returned multiplied by $0.25)
9. Please sign and date the Commercial Invoice.
Please make three copies of the Commercial Invoice and place them, along with the AWB, in the pouch on the outside of the letter or Pak. The AWB should be the document on top inside the pouch. The top copy of the AWB is for your records and should be removed prior to shipping.
REMEMBER: Complete and accurate documentation is the key to successful shipping!
